Shift: Friday - Monday | 8:00 AM - 5:00 PM
As a key member of our care coordination team, the Scheduler plays a vital role in ensuring seamless, timely, and efficient patient care. This position is responsible for organizing and managing patient visit schedules while maintaining clear and proactive communication with clinical staff, supervisors, patients, and peers. The Scheduler helps drive patient satisfaction and clinician engagement by supporting smooth operations and adapting to evolving processes. This role also ensures compliance with company policies, payor requirements, and federal regulations, making it an essential part of delivering high-quality, compassionate care.

· Speaking with patients to set up their homecare visits and verify patient information.
· Coordinating patients’ initial visits within company policy for timely initiation of care by managing multiple Epic workqueues.
· Assigns appropriate clinician coverage for all ordered services.
· Answer the telephone/emails, screen and route all incoming telephone calls/emails to the appropriate staff member or take messages according to office policy and procedure.
· Review hospitalization lists and report findings to clinicians
