This position will coordinate and provide care that is safe, timely, effective, efficient, equitable, and client-centered. Will help clients achieve wellness and autonomy by providing assistance with behavioral health, physical health and environmental factors through a variety of modalities. Will facilitate multiple aspects of care for patients own behavioral health. Position will provide direct care support to patients to ensure maintaining successful and safe living in the community independently.

• Provides at home or office, in community or by phone visits to assigned patients to monitor home conditions and supervise/support activities of daily living.
• Completes Functional Assessment and Initial Treatment Plan within 30 days of admission to program, in conjunction with patient.

• Maintains strict patient confidentiality/follows HIPAA guidelines.
• Participates in Center's operations demonstrating professional behaviors to include supporting goals of the Center and maintain awareness of resource utilization.
• Provides clinical support to physician/ARNP/ PA for complex cases requiring case management, for example, contacting doctors and other health care providers to coordinate services.
• Works cooperatively with referral agencies in coordinating services to ensure optimal consistency of service provisions.
• Maintains and develops professional relationships with all outside agencies and external customers by communicating in an efficient, professional and courteous manner.
Education:
Bachelors degree in Social Work, Human Services, or another Human Service related field required.
Experience:
Experience with providing service to individuals with a severe and persistent behavioral health needs required.
In home care settings or 1 to 2 years in behavioral health setting preferred.
License(s)/Certification(s):
Valid driver’s license when driving any vehicle for work-related reasons. Class D license required.
Knowledge/Skills/Abilities:
Ability to delegate wisely, exercise good judgment, maintain good interpersonal relationships, adapt to change, evaluate critically and think creatively. Individual will demonstrate knowledge and skills necessary to provide appropriate care to adult and geriatric patients with severe and persistent mental illnesses. Individual will demonstrate effective written and verbal skills allowing communication with physicians, co-workers, other clinic/hospital personnel, as well as the public. Position requires good organizational skills.
