As a Registered Nurse Hospice, you’ll bring compassion, clinical expertise, and a deep sense of purpose to patients and families during life’s most important moments. Working in the comfort of each patient’s home, you’ll perform physical assessments, deliver personalized care, and provide critical education and support. Collaborating closely with physicians and the care team, you’ll help create and carry out individualized hospice care plans that prioritize comfort, dignity, and quality of life. · Makes visits to the homes of patients requiring hospice nursing services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.
· Ensures compassionate, empathetic quality and safe delivery of services to patients according to care plan. Uses equipment and supplies safely, effectively, and efficiently.
· Consistently utilizes a patient-centered approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patients served in hospice.
· Delivers quality care and completes clear, concise and accurate clinical documentation of care provision in accordance with hospice conditions of participation (CoPs), various payer hospice coverage criteria, accreditation standards, professional standards for discipline/credential and the hospice plan of care.
· Maintains accurate and timely documentation of clinical records and time/travel entries.
