Assist provider and other clinical staff with obtaining basic vital information preparing participant for examination, treatment and minor surgical procedures. An extension of the provider positioned to assist with all necessary care and completion of that care. Responsible for tracking system for follow-up appointments related to office visits, laboratory tests and procedures to regularly follow participants monitoring disease status. . Complete appropriate forms or requisitions needed for referrals or lab testing.

Clinical Excellence/Patient Care
• Prepare exam rooms with necessary supplies and cleanliness to allow for an efficient day.
• Demonstrate competency in clinical skills necessary to complete basic rooming functions to include vital signs, completion of chart review for outstanding patient needs and complete all necessary documentation in participants chart consistent with rooming protocols.
• Assist provider and other clinical staff in preparing participant for examination, treatment and minor surgical procedures.
• Monitor and act on participant readiness for care, arrival, need for additional care, for example blood work, procedures or imaging.
• Prepare charts in support of the provider, in a thorough understanding of care gaps and participant needs for the visit and after.
• Assist provider and other team members accordingly within scope and training during emergency situations.
• Coordinate and schedule participant assessments with PACE provider
• May be required to draw and collect blood and other samples from participants and prepare specimens for laboratory analysis.
• Carry out clinical orders given by provider as appropriate deemed by scope of competency including but not limited to administering medications and vaccinations orally or by injection.
• Responsible for managing the tracking system for follow-up on laboratory tests and procedures to regularly follow participants to support their health.
• Responsible for assisting in managing results and follow-up as determined by provider.
• Responsible for safely administering vaccinations and medications at the direction of the provider or by protocol.
• Complete tasks as delegated by the provider within scope of responsibilities and competency.
• Provide support outside the office visit for participant inquiries and follow-up at the direction of the provider.
• Provide information to participants and family members as directed by provider
• Complete appropriate forms or requisitions needed for referrals or lab testing, including appropriate CPT/ICD-10 coding.
• Provide appropriate and timely documentation in the participants electronic medical record using standardized workflows and processes.
• Perform other duties as requested by the Clinical Manager to facilitate the smooth and effective operations of the office.
Education
• Graduate of an accredited nursing program or current CMA certification.
• High school diploma or equivalent.
License(s)/Certifications)
• Current CMA certification OR
• Current LPN license OR
• Current RN license
• BLS and Mandatory Reporter certification.
• Valid driver’s license when driving any vehicle for work-related reasons.
