Hours: Full-Time, Monday-Friday, 8am-4pm

 

The Acute Rehabilitation Program Coordinator is an integral part of a multidisciplinary collaborative effort to identify and deliver quality and cost efficient healthcare to patients on acute rehabilitation.  The employee is a liaison between persons served, the rehabilitation team, and external resources. Serves as one of the primary facilitators for patient referrals from internal and external referral sources to ARU.  In this role the rehabilitation referral coordinator will be expected to interact with discharge planners, insurance carriers, physicians, patients, family members and other related parties who may be involved with or inquiring about the rehabilitation programs. 

Responsible for providing pre-admission intake assessments for acute inpatient rehabilitation services. Also have primary responsibilities in the day to day areas of patient evaluation using the IRF-PAI, ensuring timely and accurate completion of the IRF-PAI/CARE tool. Responsible for inputting the data, and electronically transmitting the data to Medicare/UDS. Assures the documentation meets criteria for Finley and CMS regulations. 

Rehabilitation program coordinator will work closely with Rehab Nurse Manager to ensure leadership standards are met.  This will include reporting and interpreting rehabilitation data, preparing required reports, and active involvement in all CARF requirements. Responsibilities

Pre-admission Referrals/Assessments

  • Acknowledges all referrals to IRF and communicates to team.
  • Is knowledgeable of post-acute levels of care and their admission criteria, i.e. IRF, SNF, LTACH.
  • Assures IRF admission criteria are met in accordance to CMS regulations.
  • Conducts pre-admission assessment and completes the required documentation. Communicates the pre-admission assessment to rehab physician for further review and admission decision.
  • Meets referred patient/family to review the IRF program/services and identify goals and discharge plan prior to IRF admission.

Marketing/Referral Relationship Development/Volume Growth

  • Fosters excellent communication between referral sources, physicians, insurance companies, and the interdisciplinary IRF team.
  • Participates in performance improvement opportunities related to IRF referral process.
  • Assures monthly/quarterly referral tracking data is complete and accurate.

Care Coordination

  • Assists team in determining appropriate LOS and discharge planning.
  • Participates in weekly care conferences with the treatment team and the persons served.

Insurance Reviews/Financial management of the stay

  • Communicates with external sources for insurance approvals and denials.
  • Submits completed Rehabilitation Patient Assessment instrument (IRF-PAI) to CMS on the timetable established by CMS. Assures documentation meets criteria for CMS regulations.
  • Facilitates the gathering of appropriate information and documentation to coordinate ongoing insurance reviews.
  • Verifies that referred patient has IRF benefits and performs all required prior authorization authorizations prior to IRF admission.

Qualifications

Education:

  • Graduate of either an accredited Nursing program or a graduate of a Physical or Occupational Therapy program.

Experience:

  • Computer experience required. Must be able to use Word.
  • Flexibility in learning and applying new methods of technology.
  • Strong organizational skills.

License(s)/Certification(s):

  • Active, unexpired license - RN, P.T., O.T., or Speech Pathologist - in the state of Iowa.
  • Valid driver’s license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities:

  • Writes, reads, comprehends and speaks fluent English.
  • Must be able to communicate effectively (written and orally) with people of diverse professional, educational and lifestyle backgrounds.
