Clinical Excellence/Care Delivery
• Home Care Aide Services
• Provide physical support to assist clients to perform daily living activities such as bathing, dressing, toileting, standing, walking, or light exercise.
• Reminds clients to take medications.
• Uses safety measures to prevent incidents and instructs clients in home safety measures.
• Notifies RN on same day of changes in client’s condition and documents notification.
• Demonstrates competency in all skills assigned by RN.
• Gives clients information on community resources and notifies supervising RN of action.
• Encourages clients to complete tasks as able for themselves.
Homemaker Services
• Completes assigned household tasks in courteous, efficient manner.
• Complies with client’s needs in completing tasks according to their instructions.
• Observes other tasks that could contribute to client’s safety and makes suggestions to RN regarding care plan.
Clerical Support
• Provides clerical support for other public health programs by answering telephone calls, making appointments, entering data into computer systems, assisting with equipment requests and maintenance of loan closet.
Education:
• High School diploma, GED, or high school equivalent.
• Certified Nurse Assistant (CNA) preferred
Experience:
• Home Care Aide experience preferred.
License(s)/Certification(s):
• Valid driver’s license when driving any vehicle for work-related reasons.
