RN Clinical Capacity Expediter - Transfer Center

Days, Monday-Friday, Full-time

Onsite: Des Moines, IA

 

This position plays a vital role in ensuring the smooth and efficient management of patient flow within the hospitals.  This position is responsible for supporting coordination of patient flow, monitoring bed availability, and facilitating timely transitions to optimize hospital capacity and ensure high-quality patient care across the system. This position works collaboratively with clinical teams and other hospital departments to ensure patients transition to appropriate care settings while minimizing delays and avoiding bottlenecks in the patient care process.

Responsibilities

 

Capacity Evaluation/Review:

  • Monitors hospital bed status and capacity across multiple facilities within the system.
  • Monitors and manages worklists to identify and address care progression delays with prioritization of patients identified for early (before 1200) discharge.
  • Ensures accurate patient class, in collaboration with Utilization Management, in support of compliance and revenue integrity functions.
  • Assists providers with documentation assistance surrounding date of discharge or medical readiness upon request
  • Collaborates in bed huddles, contributing to team discussions on patient capacity, discharge planning, and optimizing bed utilization to enhance operational efficiency.
  • Promotes proactive inpatient care progression and escalate tasks that could impact care or discharge timeline by expediting, prioritizing, mediating, and resolving clinical issues.
  • Facilitates problem-solving of issues related to patient flow, transfer, discharge and care progression in partnership with clinical/non-clinical teams focusing on quality, safety, and patient experience.

Operational Partcipation:

  • Participates in meetings, data collection, and data preparation for sharing outcomes
  • Participates in the development, implementation and ongoing evaluation of policies, processes and procedures related to Capacity Management functions as well as outside departments to assist in process improvement to ensure the meeting or improvement of organizational goals related to Capacity Management

Qualifications

Education: 

  • Bachelors Degree in Nursing

Experience: 

  • 2 year's of Care Coordination Experience

Licenses/Certifications:

  • Must be licensed as a Registered Nurse in the state which you practice and/or hold an active Compact/Multi-State RN license.

Knowledge/Skills/Abilities:

  • Excellent written and verbal communication
  • Highly organized
  • Proficient with technology including Microsoft products and EPIC
  • Able prioritize prioritize and complete work with minimal supervison
