Shift: Monday - Friday | 8:00 AM - 4:30 PM (+ rotating weekends)
As a Home Health Physical Therapist, you’ll bring expert care directly to patients where they feel most comfortable—their own homes. Using your clinical skills in physical assessment, therapy intervention, and patient education, you’ll play a key role in helping individuals regain mobility, independence, and confidence. You’ll collaborate with physicians and the broader care team to create and implement personalized care plans that support recovery, prevent hospital readmissions, and improve overall quality of life. This role combines hands-on therapy with meaningful connections, allowing you to make a lasting impact—one patient at a time.

· Makes visits to the homes of patients requiring home health physical therapy services and assumes responsibility for an ongoing interdisciplinary assessment of the patient.
· Performs initial and comprehensive assessments on all patients in accordance with agency policies and procedures.
· Evaluates and assesses the patient’s level of functioning by applying appropriate tests.
· Treats clients to relieve pain, develop and restore function and maintain maximum performance using appropriate physical therapy treatments, modalities and procedures as stated in the physical therapy plan of care.
· Initiates diagnostic, preventative and rehabilitative procedures as appropriate to the patient’s care delivery
