Overview

The Qualified Mental Health Professional (QMHP) is responsible for the promoting and implementing Abbe Center for Community Mental Health’s mission by participation in development, implementation, and evaluation of services provided for the clients of crisis services.

 

Our QMHP will provide behavior supports to the individuals in crisis services by engaging with the individual daily and completion of assessments. The engagement will assess for safety, presenting crisis issues and needs, include skills training services and supporting the individual on their treatment and discharge goals.  The QMHP will work collaboratively with the crisis staff, on-site agencies and other treatment team members for the individual including contact with community providers and family members or natural supports. The QMHP will be strengths-based model that will help develop and build skills of those in crisis.

 

The QMHP supports optimal care that requires specialized knowledge, critical thinking skills, has foundation of knowledge for therapeutic interventions that supports the principles of psychological, social, and physical needs. The QMHP demonstrates the attributes of servant leadership, communicates exceptionally, and motivates others to achieve outcomes through work with multidisciplinary teams. This person is responsible for staff supervision and being on call as needed. Works in collaboration with program leadership to identify areas of improvement regarding practice, policies and processes.

Responsibilities

Leadership

·        Builds trusting, collaborative relationships with individuals in crisis services and establishing collaborative relationships with staff, peers, physicians, and administration to advance the core crisis services.

·        Applies HR practices fairly and consistently.

·        Commits to the hours needed to achieve outcomes and develop others to assure round-the-clock leadership. Requires flexibility with hours worked and shifting priorities as the environment changes,                     without losing sight of goals/outcomes.

·        Completes crisis assessments, provides diagnosis of individual in services as outlined by program requirements.

·        Develops treatment plan with individual in services and provides with a wholistic person-centered services through the development, planning, implementation, and evaluation of individual needs.

·        Meets with individuals face to face, exceptions may be made as directed and approved by Director.

·        Provides crisis assessment and planning, crisis linkage and follow-up, and assist individual with reaching stabilization during the stay.

·        Maintains an empathic, yet professional, relationship with children and families and strives to keep families engaged in all services

·        Follows-up on clients concerns as soon as possible.

·        Notifies Director of concerns that have the potential to escalate or lead to Risk Management concerns.

·        May be requested to travel to conferences and meetings as an agency representative. Must be able to plan to attend these as required.

·        Ensures compliance with licensure laws and regulatory agencies’ standards.

·        Serves as liaison between the resident and significant others involved in the client’s care (i.e., family, advocates, probation officer, guardian/conservator, etc. as needed.

         Maintains accurate records that includes ICD-10 diagnosis completion and documenting assessments daily.

 

Professional Development of Team Members

·        Fosters development of unit leadership team.

·        Ensures a thorough and adequate orientation program for new staff.

·        Ensures the adequate competency training and continuing education programming is done to meet identified staff development needs.

·        Supports department and agency shared governance infrastructure and staff engagement.

·        Supports continuous staff development and career goal achievement.

Qualifications

 

Minimum Requirements

Identify items that are minimally required to perform the essential functions of this position.

Education:

 

 

 

·        Successful completion of an accredited Social Work, Mental Health Counseling, Marriage Family and Therapy or nursing degree with additional certification.

·        Independent licensure

Experience:

 

 

 

 

·        Two years of clinical leadership with recognized knowledge and expertise in caring for specific resident populations emphasis in human services/mental health/intellectual disabilities, and or gerontology

License(s)/Certification(s):

 

 

 

 

·        Possess current licensure in respective discipline.

·        Valid driver’s license when driving any vehicle for work-related reasons.

Knowledge/Skills/Abilities:

 

 

 

 

·        Writes, reads, speaks, and comprehends fluent English

·        Ability to direct others and to exercise judgment in problem-solving scenarios

·        Ability to lead and/or participate in multidisciplinary teams

·        Possess a comprehensive knowledge of general nursing theory and practice and an expanded knowledge of current clinical and specialized resident care concepts.

·        Possess a broad and varied knowledge base that is continually enhanced through self-directed study and participation in educational opportunities.

·        Possess knowledge of performance improvement and quality standards.  

·        Ability to complete basic computer course through in-hospital training; develops and maintains necessary computer skills including Word and Excel.

Other:

 

 

 

·        Use of usual and customary equipment used to perform essential functions of the position.

·        Pre-employment participation in mandatory drug testing, and thereafter as set forth by AbbeHealth policy and the Drug Free Workplace.

·        Valid Iowa Driver’s License or ability to get one shortly after employment.

·        Results of Motor Vehicle Record (MVR) must meet the requirements for violations as set forth in AbbeHealth’s Vehicle Fleet Driving Program.

·        Carry current automobile insurance in accordance with AbbeHealth’s policies.
