The Clinical LPN/RN – Family Medicine Float Nurse provides high-quality, patient-centered nursing care across multiple primary care clinics in the Des Moines region. This position is responsible for supporting family medicine teams by floating between locations based on staffing needs, ensuring seamless clinical operations and continuity of care. The nurse will deliver direct patient care, assist with procedures, support providers, and educate patients and families in accordance with clinic protocols and nursing standards.
Deliver professional nursing care to patients of all ages under provider supervision in a family medicine setting.
Float between designated family medicine clinics to provide staffing support based on scheduling needs and provider coverage.
Perform clinical duties including rooming patients, collecting vital signs, documenting medical history, administering medications and immunizations, and assisting with procedures.
Conduct phone triage, respond to patient messages, and provide education regarding health conditions, medications, and treatment plans.
Coordinate lab work, imaging, referrals, and follow-up appointments as ordered by the provider.
Monitor patient conditions, report changes to providers, and respond to urgent needs as appropriate.
Document all patient interactions accurately in the electronic health record (EHR).
Maintain infection control and safety standards in accordance with clinic policy.
Adapt quickly to varying clinic environments, workflows, and team dynamics.
Current LPN or RN license in the State of Iowa (unencumbered)
BLS (Basic Life Support) certification
Valid driver’s license and reliable transportation for travel between clinics
Previous clinical experience in a medical office preferred.
Computer data entry experience.
Mandatory Reporter certification.
CPR certification.
Strong interpersonal skills.
Strong computer skills. Ability to work as a team member.
Ability to understand and apply guidelines, policies and procedures Able to write, read, comprehend and speak fluent English. Strong verbal and written communication skills.
