Shift: Monday-Friday 8a-4:30pm

• Assess patients medical and behavioral health needs

• Review all patient medications and check for potential drug interactions

• Ensure that the patient all providers who prescribe medications for the patient have an accurate medication list and holistic treatment plans

• Refer patients to community resources

• Create an integrated health treatment plan with patient engagement

• Follow up with referral sources to ensure that patients and other providers are following their treatment plan

• Obtain health information from patients including but not limited to height, weight, waist circumference, allergies, current and previous medications, and smoking

• Responsible for ensuring that one’s caseload is meeting health performance indicators

• Enter patient health information into computer system

• Assist patients in completing patient assistance and other public assistance forms

• Assist patients in completing Integrated Health Home enrollment forms

• Assist patients in completing health questioners

• Provide direct services to patients as needed including but not limited to transportation

• Assist patients in completing quality assurance surveys

• Day travel to satellite clinics may be required. Mileage for travel will be paid

REQUIRED QUALIFICATIONS:

• License RN or higher

• Six month’s experience working in a medical clinic, home health, hospital, or nursing home as an employee or intern.

• A valid driver’s license, auto insurance and reliable transportation

• Computer Skills

PREFERRED QUALIFICATIONS:

• Experience providing behavioral health services
