Ensures quality and safe delivery of infusion services to patients according to care plan.  Receive, evaluate, and process physician orders.  Monitor and document therapy for safety and effectiveness.  Collaborate with medical care team and patient to ensure best outcomes are achieved.   Performs all duties required and assigned in a timely manner. Maintains professionalism at all times and exemplifies the FOCUS values

Responsibilities

Medication Review and Dispensing:

  • Review and interpret physician orders to ensure accuracy and appropriateness.
  • Dispense medications in compliance with legal and regulatory standards.
  • Contact physicians as needed for order clarification and consult on drug therapy with healthcare professionals.

Drug Therapy Monitoring:

  • Assess the therapeutic appropriateness of prescribed medications.
  • Monitor for therapeutic duplication, dose appropriateness, and adherence to drug regimens.
  • Evaluate potential drug, food, or diagnostic test interactions and disease limitations to drug use.
  • Utilize laboratory and clinical monitoring methods to detect drug effectiveness, side effects, toxicity, or adverse effects.
  • Maintain comprehensive clinical records.

Pharmacy Operations Oversight:

  • Supervise pharmacy personnel to ensure accurate dispensing per physician orders.
  • Manage the drug control system, including receipt, storage, packaging, preparation, and dispensing of medications.
  • Ensure proper labeling and preparation of prescriptions for delivery.

Qualifications

Education:

  • Bachelor of Science or PharmD. degree.
  • Graduate of an accredited school of pharmacy.

Experience:

  • One year experience in retail or clinical setting

License(s)/Certification(s):

  • Current license to practice pharmacy in the state(s) where services are provided.
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.

Knowledge/Skills/Abilities:

  • Sufficient knowledge, training, and expertise in pharmacotherapeutics relative to infusion medications as well as specialty medications.
  • Strong interpersonal skills.
  • Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
  • Knowledge of professional pharmaceutical principles and practices.
  • Ability to work as a collaborative team member.
  • Ability to understand and apply guidelines, policies and procedures
  • Basic knowledge of computer applications (i.e. Word, Excel) and operations.
  • Ability to maintain confidential information
