Responsibilities

  • Provide Direct Patient Care and Symptom Management
    Deliver compassionate, high-quality nursing care in patients' homes according to individualized hospice plans of care. Conduct comprehensive assessments, manage symptoms using evidence-based tools, and implement physician-ordered treatments to promote comfort and dignity at the end of life.

  • Coordinate Interdisciplinary Care and Communication
    Collaborate closely with patients, families, and interdisciplinary team members to ensure coordinated care and goal-aligned decision-making. Supervise hospice aides and LPNs, and communicate care updates effectively to providers and team members.

  • Maintain Clinical Excellence and Documentation Standards
    Accurately document all patient care activities in compliance with hospice regulations and professional standards. Participate in quality improvement initiatives, peer reviews, and staff training while maintaining infection control practices and productivity expectations.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses

Experience:

  • One year nursing experience caring for similar patient population as to be assigned.

License(s)/Certification(s): 

  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.
  • Mandatory Reporter: Child & Dependent Adult Abuse.
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
