Overview

The Stroke and Sepsis Program Manager coordinates the care of patients experiencing stroke and stroke-like symptoms and care of patients at risk for or diagnosed with sepsis.

 

Mentors and role models high quality care of patients experiencing stroke or sepsis and compliance with the core measures.

  • Leads initiatives and improvements between departments and differing levels of care to develop consist, high quality care to meet standard measures.
  • Leads work to obtain/maintain primary stroke certification.
  • Acts as a resource to all UnityPoint Health, rural hospitals and state stroke task forces.
  • Works to continually lead improvement processes and incorporate the latest evidence-based guidelines and standards into practice.

 

The successful program manager will strive for improved outcomes/measures, develop positive working relationships with the multidisciplinary network of stakeholders, and maintain a proactive approach to preventing and treating stroke and sepsis to decrease possible disability or death.

Responsibilities

Clinical Outcomes and Data Management 

 

  • Leads the Market Committees related to stroke and sepsis.
  • Leads the stroke certification preparedness, surveys and post survey action planning.
  • Leads project teams to define, improve, and evaluate processes related to patient care. Uses principles of performance improvement to standardize and hardwire changes.
  • Evaluates care coordination across the continuum to obtain data and improve re-entry into the community and enhance long-term outcomes.
  • Reviews and assists with processes to improve coordination of care for patients experiencing stroke or sepsis, including meeting or exceeding stroke certification requirements.
  • Coordinates and implements clinical summaries/statistics, and reports for quality improvement.
  • Performs required abstraction of the clinical data and case reviews based on quality indicators to assure consistency of care and accurate, data collection to meet the data abstraction and reporting deadlines.
  • Analyzes data, communicates with key leaders and clinicians to improve results.
  • Acts as an interdepartmental and community liaison to positively impact patient care provided by the interdisciplinary team.

Outreach/Education

  • Collaborates with UnityPoint Health stroke and sepsis leaders to develop and implement education for team members and public.
  • As UPH-Dubuque liaison, develops relationships with external groups and agencies such as Iowa Stroke Task Force, American Stroke Association, and affiliate hospitals to influence improved stroke or care in the community.
  • Market representative to the UnityPoint Health clinical effectiveness work.
  • Coordinates and or participates in clinician and community education to achieve and maintain current evidence-based practice and stroke knowledge.
  • Acts as a resource to all UPH, rural affiliates, and the community to improve and enhance care of the patients experiencing stroke or sepsis.
  • Organizes and participates in community activities and opportunities to promote knowledge of stroke and sepsis prevention and warning signs and symptoms.

Clinical Care Evaluation and Reporting

  • Supports the clinical review and submission of quality data related to assigned areas, Examples National Database for Nursing Quality Indicators (NDNQI), Centers for Medicaid and Medicaid Services (CMS)
  • Supports clinical leadership review of cases for patient outcome and safety concerns.

Qualifications

 

Minimum Requirements

Education:

 

 

 

 

·         Bachelor’s degree or higher in nursing or related field.  

·         Maintain no less than 8 hours of education focusing on cerebrovascular disease and treatment of acute stroke patients annually.

 

·         Master’s degree in nursing or related field preferred.

Experience:

 

 

 

 

·         Minimum of 2 years’ nursing experience in caring for patients who are critically ill. 

·         Previous experience with quality improvement processes and analytical skills.

·         Experience caring for patients with stroke or sepsis.

License(s)/Certification(s):

 

 

 

 

·         Current Iowa License to practice as a RN.

·         Valid driver’s license when driving any vehicle for work-related reasons.

·         Specialty RN certification preferred (i.e., SCRN or CNRN)

Knowledge/Skills/Abilities:

 

 

 

 

·         Read, writes, speaks fluent English.

·         Must be able to use technology to manage data, evaluate patient care processes and communicate within UPH-DM and the community at large.

·         Must have proven experience in leading change among diverse groups or across departmental boundaries.

 

Other:

 

 

 

·         Use of usual and customary equipment used to perform essential functions of the position.
