Day Shift:

  • 8:00 AM - 5:00 PM
  • 3 Days/Week
  • Weekend + On-Call Rotation
  • 2 Holidays/Year

Responsibilities

  • Deliver In-Home Occupational Therapy - Provide skilled occupational therapy services in patients’ homes, including assessments, treatment planning, and interventions aligned with physician orders and individualized care plans.
  • Coordinate Care and Ensure Quality Outcomes - Collaborate with interdisciplinary team members, patients, and caregivers to develop, implement, and update care plans that promote patient independence, prevent hospitalizations, and achieve measurable health outcomes.
  • Maintain Clinical Documentation and Compliance - Ensure accurate, timely documentation and compliance with regulatory, accreditation, and agency standards, including participation in quality improvement, training, and performance initiatives.

Qualifications

Education:

  • Graduate of an occupational Therapist education program accredited by the Accreditation Council for Occupational Therapy Education (ACOTE) of the American Occupational Therapy Association, Inc. (AOTA), or successor organizations of ACOTE 
  • Eligible to take, or has successfully completed the entry-level certification examination for Occupational Therapists developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT) 

Experience:

  • One year experience caring for similar patient population as to be assigned. 

License(s)/Certification(s):

  • Licensed or otherwise regulated, if applicable, as an Occupational Therapist by the state in which practicing, unless licensure does not apply in the practicing state. 
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.   
  • Mandatory Reporter: Child & Dependent Adult Abuse. 
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
