Responsibilities

  • Provide Direct Patient Care
    Deliver compassionate, hands-on personal care and companionship to hospice patients under the supervision of the RN case manager, including hygiene assistance, grooming, toileting, and light housekeeping.

  • Support Care Coordination and Communication
    Report changes in patients’ conditions, contribute to accurate and timely documentation, and help maintain continuity of care in alignment with individualized care plans and hospice protocols.

  • Demonstrate Professionalism and Regulatory Compliance
    Uphold organizational values, safety standards, and regulatory requirements while maintaining licensure, completing required education, and ensuring compliance with healthcare laws and guidelines.

Qualifications

Education:

  • Completion of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 without compensation for Home Care Aide/CNA work since completion

License(s)/Certification(s):

  • Valid driver's license when driving any vehicle for work-related reasons.
  • CPR Certification.
  • Valid Mandatory Reporter course completion by state(s) requirement.
  • Valid automobile insurance in accordance with sate and/or organizational requirements.
