Patient Care
Consistently uses a holistic approach considering physical, psych/social, spiritual, educational, safety and related criteria appropriate to the age of the patient served in assigned service area. Ensures quality and safe delivery of services to patients according to plan of care.
Care Coordination
Conducts assessments of the social and emotional factors related to the patients/clients illness, need for care, response and adjustment to care. Completes appropriate plan of care and interventions according to scope of practice.
Clinical Excellence
Assesses the relationship of the patient’s medical and nursing requirements to the patient’s home situation, financial resources and available community resources. Provides information and appropriate referral action to obtain available resources to assist in resolving patient/family issues.
Education: Baccalaureate degree in social work, psychology, sociology, or other field related to social work.
Experience: One year clinical experience in health care.
License(s)/Certification(s):
Valid Mandatory Reporter course completion by state(s) requirement
CPR Certification; maintain a valid Basic Life Support (BLS) Healthcare Provider Card and re-certification
Knowledge/Skills/Abilities:
Strong verbal and written communication skills.
Ability to take initiative and exercise independent judgment, decision making and problem solving expertise.
Ability to work as a collaborative team member.
Ability to understand and apply guidelines, policies and procedures.
Person Centered Care (PCC) course completion within first 12 months of hire and annual completion of competency validation activities.
