Congestive Heart Failure Nurse Coordinator

Full-time, 0.8 FTE

7:30am-3:00pm; Varied Hours

An exempt position responsible for coordinating all services required to provide multidisciplinary, quality assured, and cost-effective care to patients who have heart failure. 

The Heart Failure Nurse Coordinator will serve as a resource for Heart Failure patients, physicians and others serving in the role of educator, and quality improvement champion. 

An initial contact with the patient/family will be made to establish a relationship and understanding of the Heart Failure care management. 

The nurse will assess the patient’s status and provide evidenced-based knowledge and skills to patient and family that foster self-care efforts and increase patient satisfaction. 

Telephonic monitoring will be initiated with 7-14 days post –discharge and continued assessment will determine follow-up per patient need.

Responsibilities

This position is 60% bedside patient care, 30% care coordination, and 10% basis UPH performance criteria.

 

Patient Care

  • Serves as primary contact for physicians and other care providers, responsible for managing patient care needs and progress, care plan development and discharge planning.
  • Follow all heart failure inpatients on all nursing units.
  • Perform daily assessment to evaluate that appropriate care plans/protocols are in place and being followed by appropriate members of multi-disciplinary team including documentation of findings.
  • Collects and synthesizes patient data based on assessment including but not limited to health and medical history, physical examination, review of diagnostic tests/procedures and assessment of preventive health screening.
  • Develop an individualized evidence-based care plan that includes patient identified self-management goals.
  • Ensures that and supports patient to actively participate in identified self-management care goals.
  • Navigate the heart failure patient through the inpatient setting and ensures discharge planning matches complexity of the disease process.
  • Perform chart audits as appropriate to determine compliance with heart failure clinical care protocols.
  • Evaluates learning needs and abilities of all HF inpatients and adapts education to meet those needs.
  • With unit case manager, evaluates post discharge level of family/significant other support and adapts education for these key care givers.
  • Perform very consistent assessments of patients to assure that the education plan created is being carried out and work with staff nurses, pharmacists and other multidisciplinary partners to close identified gaps.
  • Take a very active role in supporting preparation for discharge and follow up.
  • Assist in discharge planning with case management, bedside RN; physician, and family or key care giver.
  • Conduct assessments with Professional Development and nursing staff regarding effectiveness of heart failure educational materials.

Care Coordination

  • Resolves post-discharge issues needing follow-up & provide post-discharge homecoming calls to follow-up with high-risk heart failure patients.
  • Coach and education designated patient population and family regarding chronic disease self-management and collaborate with the PCP and specialty physician regarding the patients care either in the hospital, during a homecoming call, and continued follow up phone calls.
  • Provide information regarding internal and external resources and services to help support patient upon discharge from inpatient facility.
  • Follow up on pending labs and patient’s care gaps during and upon discharge from acute care facility.
  • Coordinate care with patient and care team during transition until patient has been seen by established/new PCP provider and specialist provider.

Basic UPH Performance Criteria

  • Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse.  Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.  Takes appropriate action on concerns reported by department staff related to compliance

 

 

Qualifications

Education:

  • Must be a graduate of an accredited nursing program.

 

Experience:

  • 3-5 years of nursing experience

 

Licenses/Certifications: 

  • Current license to practice nursing in the state of Iowa/Illinois for Registered Nurse
  • BLS

  • Ability to use the computer efficiently
