Responsibilities

  • Patient Admission & Assessment: Conducts initial and comprehensive patient assessments, develops home health care plans under physician direction, and ensures interdisciplinary team involvement for optimal patient outcomes.
  • Care Coordination & Documentation: Communicates patient status, coordinates care with the healthcare team, completes OASIS documentation, and ensures timely and accurate documentation of all patient interactions.
  • Quality Care & Compliance: Provides education to patients and caregivers, adheres to clinical best practices, participates in quality assurance activities, and ensures compliance with home health regulations and safety protocols.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses 

Experience:

  • One year nursing experience, including experience in performance of OASIS, in skilled home health care 

License(s)/Certification(s):

  • Valid license as RN in state(s) where providing care
  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.   
  • Mandatory Reporter: Child & Dependent Adult Abuse. 
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
