Overview

Assists the pharmacy, nursing, and medical staff in obtaining accurate home medication lists from patients being admitted to the hospital and conveying that information electronically to the patient record.

Responsibilities

Operational Functions

  • Obtains accurate home medication histories via patient interview and phone calls to retail pharmacies.
  • Applies “best practices” when obtaining a medication list including the following:
    • Obtaining patient demographic information including, name and medical record number, physicians, and emergency contacts.
    • Documenting the name of the pharmacy where the patient has their medication filled
    • Documenting the following information for each medication (including prescription and over-the counter) listed:
      • Drug name (generic)
      • Strength or concentration
      • Formulation (extended release, controlled release, etc.)
      • Dose
      • Route
      • Frequency
      • Last time taken
    • Reconciles the home medication list to check for duplications and/or omissions before entered electronically to patient’s record.
    • Brings all discrepancies to the attention of the confirming pharmacist.
    • Contacts the patient’s emergency contact, pharmacy, or physician office to resolve all discrepancies in a timely manner.
    • Updates the home medication list in the electronic medical record.
    • Maintains a thorough understanding of computer functions needed.

Compliance

  • Completes competency requirements prior to deadlines, as outlined in hospital policies and/ or legal requirements.
  • Demonstrates familiarity with medication reconciliation by reading current Joint Commission requirements and National Patient Safety Goals.
  • Completes a competency checklist, including observing a pharmacist during a patient interview and performing supervised patient interviews before being signed off by the pharmacy manager.
  • Remains up to date on medications and hospital processes at Finley.

Professionalism

  • Interacts with patients and medical staff using strong interpersonal and communication skills.
  • Works efficiently and accurately to budget time effectively and follow procedures with minimum supervision.
  • Participates in hospital and/or departmental committees relevant to the practice area and medication reconciliation.

 

Pharmacy Technician Functions

  • Assists pharmacy technicians by preparing and distributing medication products to fulfill medication orders.
  • Ensures appropriate and adequate medication inventory by restocking, rotating stock, removing expired medications, repackaging medications, and ordering supplies when needed.
  • Answers the telephone and directs calls to appropriate personnel in a professional and courteous manner.

Education:

 

 

 

High School diploma or GED

 

Experience:

 

 

 

 

 

At least one (1) year of hospital or retail pharmacy experience.

License(s)/Certification(s):

 

 

 

 

·       Certified as Pharmacy Technician (CPhtT) per state or federal regulations.

·       Registration in good standing with the Iowa Board of Pharmacy

·       Incumbent must pass internal Omnicell and IV Competency exams.

·       Valid driver’s license when driving any vehicle for work-related reasons.
