Responsibilities

  • Patient Care & Care Coordination – Conducts in-home visits to assess and provide hospice nursing care, ensuring high-quality, compassionate care aligned with the patient’s plan of care. Works collaboratively with patients, families, and the interdisciplinary team to manage symptoms, promote comfort, and adjust care plans as needed.

  • Clinical Documentation & Compliance – Maintains accurate and timely documentation of patient care, ensuring compliance with hospice regulations, accreditation standards, and professional guidelines. Participates in quality assurance activities and peer reviews to support continuous improvement in care delivery.

  • Team Collaboration & Supervision – Acts as a key member of the interdisciplinary hospice team, communicating effectively with colleagues, physicians, and caregivers. Supervises hospice aides and LPN staff, providing guidance and support to ensure comprehensive, patient-centered care.

Qualifications

Education:

  • Graduate of State Board approved program for Registered Nurses  

Experience:

  • One year nursing experience caring for similar patient population as to be assigned

License(s)/Certification(s):

  • Valid licensed driver with automobile insurance in accordance with state(s) and/or organizational requirements.   
  • Mandatory Reporter: Child & Dependent Adult Abuse. 
  • CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
