Responsibilities

Clinical Excellence/Patient Care

  • Patient and Family Needs: Assess the needs of patients and families, provide appropriate teaching, and make necessary referrals to ensure continuity of care.
  • Care Coordination: Integrate and coordinate care for at-risk populations across the healthcare continuum.
  • Individual Care Plans: Develop personalized care plans using a collaborative approach with patients, families, and the healthcare team.
  • Quality and Safety: Ensure the quality and safe delivery of services according to the care plan, using equipment and supplies safely and efficiently.
  • Documentation: Promptly complete documentation to maintain an accurate legal record of patient care and time/travel entries.
  • Communication: Clearly and accurately communicate clinical status and plans to the interdisciplinary care team, administrative support, leaders, and physicians.
  • High-Quality Care: Deliver high-quality care using the nursing process, demonstrating knowledge and clinical skills specific to the assigned unit.
  • Holistic Approach: Utilize a holistic approach considering biopsychosocial, spiritual, educational, and safety needs appropriate to the age of patients and families.
  • Patient Advocacy: Serve as a patient advocate, educator, and central source of communication for patients, their support systems, and referral sources.
  • Interdisciplinary Collaboration: Collaborate with clinical management, physicians, and the interdisciplinary team to provide high-quality, cost-effective care.
  • Outcome-Oriented Care: Ensure care is outcome-oriented, reflecting measurable progress and goals toward discharge.
  • Advance Care Planning: Facilitate discussions to ensure appropriate care coordination and high-quality care based on patients' long-term goals.
  • Delegation and Direction: Delegate and provide direction to other healthcare providers, holding them accountable for their involvement in the care plan.
  • Critical Thinking: Utilize critical thinking to make independent judgments, maintaining responsibility and accountability for patient conditions.
  • Risk Identification: Identify risks for acute hospitalization and proactively prevent adverse events.
  • Quality Assurance: Review multidisciplinary care provision, conduct clinical reviews, and participate in peer audits as applicable.
  • Service Promotion: Assist in promoting services and products.
  • On-Call Duties: Perform on-call duties and weekend/after-hour visits on a rotational basis as assigned.
  • Additional Duties: Perform other duties as requested by supervising leaders to ensure smooth and effective operations.

Development & Patient Education

  • Professional Development: Continue professional development and involvement in activities that enhance nursing practice through networking and research.
  • Teaching Strategies: Assess and choose teaching strategies appropriate to the patient's and family's age-specific needs, culture, gender, readiness to learn, and level of comprehension.
  • Education: Provide patient, family, and caregiver education and information pertinent to diagnosis and safety issues.
  • Specialized Education: Consult appropriate resources to provide specialized education.
  • Safety Priority: Demonstrate knowledge that patient and workplace safety is a priority by preventing errors, accidents, and infections, and reporting potential or actual concerns immediately.

Qualifications

Education:

Graduate of State Board approved program for Registered Nurses. 

Complete and maintain UPH Person-Centered Care: Three opportunities completed per year required (Oct – Sept scheduled year). 

Attend all mandatory education programs. 

Experience:

Strong interpersonal skills. 

Ability to work as a collaborative team member. 

Ability to understand and apply guidelines, policies and procedures. 

Strong computer skills. 

License(s)/Certification(s):

Valid / maintained license as a Registered Nurse for all states applicable to practice.  

Valid / maintained Basic Life Support (BLS) Healthcare Provider Card with Re-certification. 

Valid / maintained Mandatory Reporter course by state(s) requirement. 

Valid / maintained driver’s license and automobile insurance in accordance with state and/or organizational requirements. 

Knowledge/Skills/Abilities: 

Writes, reads, comprehends and speaks fluent English. 

Multicultural sensitivity. 

Microsoft Office – basic computer skills. 

Customer/patient focused.  

Critical thinking skills using independent judgment in making decisions.
