Hospice & Home Care Residency for New Graduate RN's (New graduate or 12 months or less experience)

8am-5pm Monday through Friday

More about the Residency Program:

This program is meant for RN new grads looking to start their career with UnityPoint at Home.  Work side by side with a RN Case Manager in the field and extend the training and education you already have with simulations and real life experiences.  This Residency Program is a one year commitment and is a full-time position which includes full-time pay and benefits.  The program is meant to prepare new RNs for the RN Case Manager role once they have completed the program.

 

The RN Resident will learn how to plan, organize and direct home hospice care services.  Will initiate, and implement care primarily in the patient’s home environment or hospice inpatient unit setting. The Hospice RN fulfills the role of skilled communicator, educator, leader and motivator. The Hospice RN ensures that high standards of patient care are met at all times.

 

The Home Care RN uses clinical nursing knowledge, physical assessment, teaching and procedural skills to deliver high quality patient care in the patient’s place of residence. The RN contributes to the development of the home health plan of care. The RN delivers patient care directed by the physician as established in the home health plan of care that is consistent with clinical best practices and results in high quality, improved outcomes and exceptional patient experience.

  5. The changing needs of your patients and their conditions mean that you’ll be utilizing a variety of skills. If the patient is new, you’ll begin with assessments and create their care plan. As you progress in our Residency Program, your experience increases and from there you’ll gain autonomy in deciding the best care for each patient.

Program Highlights

  1. A 12-month program to smooth transition into practice.
  2. A series of thoughtfully developed classes that will enhance skills in critical thinking, assessment, leadership and clinical decision making.
  3. Participation in this program is a part of the regular work assignment.
  4. Classroom instruction specific to the clinical area using a focused case study approach to learning.
  5. Clinical Experience under the guidance of an experience Preceptor.
  6. Ongoing continuing education and professional development opportunities.
  7. Dedicated nurse educators are available both during and following the program.

 

Nurse Resident Required Program Participation during the 12 month program:

  • SIM Labs
  • Monthly educational seminars
  • Meeting touchpoints as scheduled
  • Completion of Residency Project
  • Completion of Residency Portfolio

Qualifications

Qualifications

  1. Graduate of State Board approved program for Registered Nurses.
  2. CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.
  3. Strong interpersonal skills.
  4. Ability to work as a collaborative team member.
  5. Ability to understand and apply guidelines, policies and procedures.
  6. Strong computer skills.
  7. Valid licensed driver with automobile insurance in accordance with state and/or organizational requirements.

 

QUALIFICATIONS PREFERRED:

  1. BSN or BS in health related field.
