Under the supervision of an RN, the Patient Care Technician provides direct patient care throughout all nursing areas.  Duties include assisting patients with their ADL’s, transporting patients in various types of equipment and ambulation, documentation of care and treatments done and observes patients for safety and condition changes and reports to appropriate staff.

Delivery of Care

• Assists patients with ADL’s according to hospital procedures, reports all abnormal findings and changes in patient condition to RN in charge of patient. Assists with ambulation of patients and uses assist devices as appropriate to protect patient and staff member.
• Assists with restocking of rooms and maintain a safe and clean environment.
• Follows all established infection control policies and recognizes role in providing safety to the patient.
• Directly observes identified patient(s) at all times during shift when preforming safety or suicidal observation.
• Promotes and ensures implementation of patient safety protocols (elopement precautions, suicide precautions, and aggressive behaviors).
• Obtain EKG under the direction of the Registered Nurse.
• Transfer of patients to other areas of the hospital under the direction of the registered nurse.
• Insertion of urinary catheters under the direction of the registered nurse.
• Cleaning and general housekeeping of rooms between patients.

Documentation

• Real-time EMR documentation of vital signs, weights, activity, intake and outputs, and ADL care is done in accordance with hospital and unit policies.

Effective Communication

• Demonstrates good verbal and written communication skills with patients, families, healthcare team members and other customers.
• Verbally presents factual, concise, and timely information and/or patient care data to the RN, the healthcare team, patients and/or family members in a professional manner.
• Hand-offs will be done with attention to detail and with safety as a high priority. Hand-offs are utilized to communicate patient treatment and assessment information, patient education needs, goals of care and other quality care measures at each change of caregiver or setting of care.
• Collaborates with healthcare team to provide patient care.
  • Consults with RN and/or other healthcare team members, as appropriate.
  • Obtains instruction and supervision, as necessary, when implementing new or unfamiliar tasks or duties.
  • Complies with established department routines, policies, and procedures.
