Admissions Coordinator

Monday - Friday 8a - 4:30p
Works with the Program Managers of New Leaf and the Knolls, Assessors at other sites, Referral Sources and Other Organizations in the community to coordinate assessments and admissions. Manages the waiting list for New Leaf and the Knolls including following up with clients and keeping them engaged while waiting for a bed to become available. Completes Assessments for clients at New Leaf and Updated Assessments for clients at New Leaf and the Knolls. Works with the Assessors to ensure clients are placed at the appropriate level of care and that the assessment provided has a sufficient amount of information to justify level of care.

ENSURES PATIENTS RECEIVE COMPREHENSIVE SERVICES FROM INITIAL CONTACT THROUGH ADMISSION · Works with the Program Managers of New Leaf and the Knolls, Assessors at other sites, Referral Sources and Other Organizations in the community to coordinate assessments and admissions. · Completes Assessments for clients at New Leaf and Updated Assessments for clients at New Leaf and the Knolls · Coordinates with the patient, referent, etc. any needed paperwork, to be reviewed to determine appropriateness for services. · Is responsible for maintaining updated records of number of assessments, conversion to admission and outcomes. · Works with the Assessors to ensure clients are placed at the appropriate level of care and that the assessment provided has a sufficient amount of information to justify level of care · Works with other departments within UnityPlace and UnityPoint related to referrals and access to treatment services. | 40% |
EVALUATES PATIENT. PRIORITIZES CARE ACCORDING TO ACUITY AND DOCUMENTS APPROPRIATELY · Contacts the referral sources, with appropriate releases obtained, in order to initiate continuity of care through the risk classification and/or admission/treatment process. · Provides patient contact and interaction that is based upon inherent dignity, individuality, culture and beliefs of each patient. Patient care is provided in a compassionate, humane, and non-enabling manner. · Provides patient care with attention/knowledge of age specific care. · Makes appropriate referrals as necessary and facilitates the referral process for patients as appropriate. · Ensures all assessments are scheduled and covered in a timely and appropriate manner. Works well with clerical staff & supervisory staff to ensure coverage of evaluation schedule. · Provides follow up for no call/no show and patients who disengage in treatment services · Ensures follow up surveys are sent to patients and providers/referral sources and enters this information in monthly QA. · Oversees all service inquiry calls and follows up as needed. · Oversees data and outcomes related to assessments, conversion rates, and services inquiry calls. Ensures all data is logged appropriately. | 30% |
MANAGES THE WAITING LIST · Manages the waiting list for New Leaf and the Knolls and is the primary contact for waiting list questions · Follows up with clients who are currently on the waiting list to keep them engaged while they are waiting for a bed to become available · Keeps updated contact notes on the waiting list for each client · Contacts clients when a bed becomes available to see if they are able to be admitted, if not, moves to the next client on the list until the bed is filled or everyone on the list has been contacted | 18% |

Basic UPH Performance Criteria · Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines. · Demonstrates ability to meet business needs of department with regular, reliable attendance. · Employee maintains current licenses and/or certifications required for the position. · Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare. · Completes all annual education and competency requirements within the calendar year. · Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Education:

| Bachelor’s Degree in Psychology, Social Work, or Counseling preferred. |
Experience:
| At least 2 years of relevant experience in addiction treatment or admissions required. |

| Must obtain and maintain current CADC certification from Illinois Alcohol and Other Drug Abuse Professional Certification Association or valid Illinois licensure in psychology, counseling or social work within 2 years of start date | Psychology, Counseling, Social Work licensure

Other:
| Must have control over personal use of all mood-altering chemicals or behaviors. If recovering from an addiction, must currently have a minimum of 1 year of uninterrupted recovery and must actively treat this addiction by appropriate recovery practices including, but not limited to, regular attendance and participation Twelve Step fellowship or other recognized recovery processes such as active participation in church, appropriate support network, etc. Use of usual and customary equipment used to perform essential functions of the position. Work may occasionally require travel to other UPH facilities/hospitals. Required to drive your own vehicle for business purposes. |
