The Care Coordinator is an integral part of a multidisciplinary collaborative effort to identify and deliver quality and cost efficient healthcare to patients.  While the role is multifaceted, concentrated effort is given to the coordination of services for designated patient populations; ensuring adequate discharge plans are in place, and completing utilization management and quality review activities.  The care coordinator teams with unit social workers in meeting these efforts.  The care coordinator reports centrally to the Director of Behavioral Health Services, with a unit-based assignment. The care coordinator will assist in meeting care coordinator divisional needs, and helping out on the other units.  St. Luke’s C.A.R.E. delivery model (Continuity, Accountability, Relationship-based, Evidence-based) reflects a commitment to compassionate, competent care delivered by the interdisciplinary team.  Care is provided through a therapeutic relationship with the patient and family.  A spirit of inquiry is encouraged and supported.  Small tests of change and more formal research efforts are used when the evidence is not clear.  Evidence-driven protocols are utilized to assure reliable and consistent care.  The patients to be served include pediatric, adolescent, adult and geriatric.

Key Accountability

·         The associate is aware of hospital and department compliance for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse.  Brings any questions or concerns regarding compliance to the immediate attention of department or hospital administrative staff.

·         Purposefully conducts all aspects of the job in an ethical manner in support of the hospital’s commitment to ethical behavior in all areas of personal and professional activity.

·         Successfully completes hospital-wide orientation, annual hospital competencies and department/unit specific competencies.

·         For the safety of our patients and co-workers, St. Luke’s Hospital requires seasonal and job related vaccinations.  This is reviewed regularly during periodic health assessments.

·         Collaborates with patient care team members to ensure delivery of patient care, procedures, diagnostic tests, and treatments are coordinated.

·         Monitors patients' progress toward outcomes  (i.e., meets discharge screens) on all patients.

·         Communicates with physicians regarding patients' progress toward outcomes and plan of care on identified patients.

·         Communicates with patients and families the plan of care and progress toward outcomes on identified patients.

·         Ensures the discharge plan is coordinated and communicated with the multidisciplinary team on identified cases.

·         Performs utilization management and quality review activities on all patients.

·         Assures patients are receiving the appropriate level of care.

·         Mentors team members in their areas of expertise.

·         Participates in unit and hospital-wide performance improvement activities and evidenced-based practices.

·         Serves as a resource to others.

·         Performs other duties as assigned to meet the hospital’s mission.



Basic UPH Performance Criteria

·         Completes all annual education and competency requirements within the calendar year.

·         Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse.  Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff.  Takes appropriate action on concerns reported by department staff related to compliance.

Minimum Requirements

Current licensure in good standing to practice as a registered nurse in the state of Iowa.

Must possess and maintain current Basic Life Support (BLS) certification as outlined by policy or obtain certification within 3 months from the date of hire.

Preferred BSN







Prefer one (1) year of hospital clinical experience or demonstrated clinical ability.






Valid driver’s license when driving any vehicle for work-related reasons.







Writes, reads, comprehends, and speaks fluent English.

Basic computer knowledge using Word processing, spreadsheet, email, and web browser.

Knowledge of MC, MCO’s, Private Insurance policies, and regulations.






Use of usual and customary equipment used to perform essential functions of the position.

Work may occasionally require travel to other UPH facilities. May drive a UPH vehicle, rental, or own vehicle.
