The Intake Home Care Coordinator serves as the first point of contact for the UnityPoint at Home Intake Dept. The primary responsibilities of the Home Care Coordinator include monitoring for and receiving new referrals via telephone, fax and other electronic applications, reviewing referral information to identify the individual needs of each patient, accurately entering patient data in the EMR and assisting in the coordination of the admission visit in a fast paced, call center environment. The Home Care Coordinator frequently interacts with referring hospitals, physicians, board and care facilities, skilled nursing facilities, patients, families and other internal customers. The Home Care Coordinator will utilize and promote the use of excellent customer service skills when working with referral sources, patients, families, and all others necessary to facilitate a smooth and timely admission to care for our patient population.
