The Corporate Wellness RN supports the clinical functions of the Optimum Health Solutions services lines. This role provides clinical care coordination and chronic care management, serves as a phlebotomist for on-site health screenings, promotes clinical education through virtual/on-site content development and both facilitates and administers the corporate wellness annual vaccination program.

Essential Functions/Responsibilities:

Care Coordination/Chronic Care Management
· Provides telephonic, face to face, and/or virtual care coordination/chronic care support to corporate wellness participants enrolled by defined criteria · Assesses an individual’s health status including current clinical conditions, forecasted risk, preventive/disease state compliance and resources to identify needs and make recommendations for appropriate multi-disciplinary services if needed · Monitors and completes documentation of goal management and plan of care in partnership with the individual for needs and services across the health care continuum · Provides review of critical lab values and completes notification to screening participants as advised by the Optimum Health Solutions Medical Director. · Completes daily call logs, compliance documentation, and additional reporting as assigned by the Corporate Wellness Manager · Works closely with health coaches for lifestyle management and collaborative decision making regarding an individual’s program placement · Maintains proficiency in technology platform for data analytics, electronic documentation and data sharing when applicable · Other duties as assigned by the Manager of Employee & Corporate Wellness | 25% |
Corporate Wellness Screenings/Phlebotomy
· Completes screenings effectively and efficiently i.e. phlebotomy and biometric measurements · Performs patient lab testing (venipuncture) and follows quality control in compliance with hospital lab procedures/policies · Assists with other roles in the screening process (i.e. check-in) at screenings as needed · Compiles data as needed from employer/commercial screenings in a timely manner and maintains screening records while following HIPPA guidelines · Maintains proficiency in technology platform for electronic data entry and transmission · Other duties as assigned by the Manager of Employee & Corporate Wellness | 25% |
Education
· Provides support to client coordinators in making appropriate recommendations regarding client aggregate risk after reviewing health screening results · Assists as needed in development of custom clinical education materials including, but not limited to virtual learning modules, on-site presentations, and/or employer/commercial campaigns. · Other duties as assigned by the Manager of Employee & Corporate Wellness | 15% |
Corporate Wellness Vaccination Program
· Serves as point of contact for flu vaccination program · Facilitates marketing and correspondence with potential and confirmed flu vaccine clients · Manages client scheduling and completion of the flu vaccine program administrative checklist · Administers all on-site vaccinations in accordance with hospital/clinic policies and procedures · Appropriately communicates CDC vaccination information with participants and corporate clients · Completes appropriate electronic documentation of provided vaccinations and works closely with data analyst for vaccination participant tracking and client billing · Other duties as assigned by the Manager of Employee & Corporate Wellness | 25% |
Basic UPH Performance Criteria
· Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines. · Demonstrates ability to meet business needs of department with regular, reliable attendance. · Care Coordinator maintains current licenses and/or certifications required for the position. · Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare. · Completes all annual education and competency requirements within the calendar year. · Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Education:
| Graduate of an accredited nursing program | BSN Bachelor’s Degree in Nursing (BSN)

Experience:
| 3 years of clinical experience in focused areas working with multidisciplinary teams

License(s)/Certification(s):
| Must possess and maintain licensure in good standing as a Registered Nurse in state of Illinois
|
|
Knowledge/Skills/Abilities:
| Writes, reads, comprehends and speaks fluent English. Basic computer knowledge using word processing, spreadsheet, email and web browser.
