To assist the pharmacy, nursing, and medical staff in obtaining accurate home medication lists from patients being admitted to the hospital and conveying that information electronically to the patient record.

ASSIST THE PHARMACY, NURSING AND MEDICAL STAFF IN OBTAINING ACCURATE HOME MEDICATION LISTS FROM PATIENTS ADMITTED TO THE HOSPITAL.

• The basic function will be to obtain accurate home medication histories by patient interview and phone calls to retail pharmacies.
• The technician must apply “best practices” when obtaining a medication list including the following:
• Obtaining patient demographic information including, name and medical record number, physicians, and emergency contacts.
• Document the name of the pharmacy where the patient has their medication filled
• For each medication (including prescription and over-the counter) listed, document the following information
• Drug name (generic)
• Strength or concentration
• Formulation (extended release, controlled release, etc.)
• Dose
• Route
• Frequency
• Last time taken
• The home medication list must be reconciled to check for duplications and/or omissions before entered electronically to patient’s record.
• All discrepancies must be brought to the attention of the confirming nurse and provider via electronic comments on the HHS med list or direct communication if available.
• The incumbent may attempt to contact the patient’s emergency contact, pharmacy, or physician office to resolve all discrepancies in a timely manner

ACCURATELY CONVEYING PATIENT MEDICATION HISTORY ELECTRONICALLY INTO THE HOSPITAL DATABASE .

• The incumbent will update the computerized home medication database (HHS system).

The incumbent must have a thorough understanding of computer functions needed.COMPLETES TRAINING AND CONTINUES TO BUILD MEDICATION AND HOSPITAL PROCESS KNOWLEDGE

• The technician completes competency requirements prior to deadlines, as outlined in hospital policies and/ or legal requirements.
• The incumbent will become more familiar with medication reconciliation by reading current Joint Commission requirements and National Patient Safety Goals.
• The incumbent will be required to complete a competency checklist, including observing a pharmacist during a patient interview and performing supervised patient interviews before being signed off by the pharmacy manager.
• The technician will remain up to date on medications and hospital processes at MMCI.

DEMONSTRATES EFFECTIVE COMMUNICATION SKILLS AND COLLABORATION WITH PATIENTS AND MEDICAL STAFF

• The technician must have strong interpersonal and communication skills to interact with patients and medical staff.
• The incumbent must be able to effectively budget time, work efficiently and accurately, and follow procedures with minimum supervision.

Education:

• High School Diploma preferred

Credentials: 

• Certified as Pharmacy Technician (CPhtT) within 2 years of registration with the Illinois Division of Professional Regulation if hired on Jan 1, 2008 and after.
• This position must keep a current pharmacy technician license for the State of Illinois or show evidence of having applied. 
• Incumbent must pass internal competency exam. Experience: 
• 1 year retail pharmacy experience required
