Overview

Under the direct supervision of the Residential Manager, provides comprehensive Case Management and Psychosocial Rehabilitation services for adults with serious and persistent  mental illness.  These services include but are not limited to assessments, treatment planning, supportive counseling and linkage with various community resources.

Responsibilities

PRINCIPLE DUTIES AND RESPONSIBLITIES: Completes all necessary documentation in a timely and accurate manner and in compliance with established standards of the State of Illinois and Robert Young Center.Accurately enters all charges within 24 hours of delivering the service.Meets established standards for productivity.1st and 2nd shift full-time staff will have 102 billable hours of service per month. 3rd shift full-time staff will have 20 billable hours of service per month. 1st and 2nd shift part-time staff will have 41 billable hours of service per month. 3rd shift part-time staff will have 10 billable hours of service per month. PRN staff do not have a billable hour requirement.Assists in completing functional assessments and gives input into the treatment planning process.Teaches and assists with activities of daily living in home and in the community. Uses cognitive rehabilitation, in-vivo skills practice and role rehearsal (i.e. psychosocial rehabilitation).

Qualifications

EDUCATION, TRAINING & EXPERIENCE: Bachelor’s Degree in Human Services, or related field, or High School Diploma with a minimum of (5) years supervised experience in mental health field.KNOWLEDGE, ABILITIES & SKILLS: Demonstrates an ability to apply critical thinking and problem-solving abilities to a range of client situations; has the ability to accurately collect, assess, and report information; can develop treatment plans and interventions with clients; possesses interpersonal skills, professional attitude and understands the ethical standards of behavioral healthcare. Is able to recognize signs of suicidal behavior and assess for lethality.LICENSES, REGISTRATIONS AND CERTIFICATIONS: Valid Driver's license and auto insurance; CPR certification, First Aid certification, and Nonviolent Crisis Intervention (CPI) training can be obtained during employee training period
